Children enrolled in the SNG Dance Program will participate in one class per week. Classes are 30 minutes in length and consist of a warm-up, age-appropriate dance activities and cool down. Please describe below any injuries, health problems or limitations which might inhibit or limit your child’s participation:

Occasionally photos or videos of various classes are used for local and national marketing purposes. If you do not want your child’s picture taken, please check the appropriate box below. NOTE: Names are not disclosed without specific permission.

Agreement to Company Policies

• RETURNED CHECKS - There is a $30 fee on all returned checks (electronic and paper), as our bank charges us when a check does not clear for this reason.
• FIFTH WEEKS - Tuition fees are based on four-week months, but to make up for holidays and keep tuition consistent each month we do meet on 5th weeks when they occur. We do not meet during the weeks of Thanksgiving, Christmas, New Year’s, and Spring Break.
• ATTENDANCE - Monthly payments are required regardless of the child’s attendance. If you are planning for your child to be away from school, please notify our office in advance, and tuition will be prorated for your convenience.
• WITHDRAWAL - To withdraw your child, you must call or e-mail our office. If you fail to notify us directly, billing will continue to accrue for which you will be responsible. Your child will not be dropped for nonpayment unless a parent contact has been made. Please be courteous and provide us with a two-week notice if you plan to remove your child from SNG Dance. You may do so by calling 601-856-2303 or e-mail at info@stretchandgrowfit.com.
• MINIMUM ENROLLMENT - A minimum enrollment of eight children is necessary for a class to be scheduled. Classes will be cancelled with notice if this should occur.

Payment Terms: I hereby authorize Stretch-n-Grow Dance to charge my account as indicated above in consideration for professional services rendered (i.e. children’s dance classes). I understand that automatic payments will be drafted on the first day of each month. Bankdraft payments will be prorated if necessary during the first month with subsequent drafts on the first day of each month. Credit card payments will be charged in full immediately for the first month with subsequent payments on the first day of each month. Credit card payments will be prorated during the second month if necessary. I understand that a two-week written notice (via hand-written letter or email) is required to cancel monthly recurring payments.
