Health Disclosure

Children enrolled in cheerleading classes will participate in one session per week. Each class will consist of a warm-up, arm movements, jumps, cheers, chants, dance, and cool-down. Please describe below any injuries, health problems or limitations which might inhibit or limit your child’s participation:

Photos

Occasionally photos or videos of various classes are used for local and national marketing purposes. If you do not want your child’s picture taken, please check the appropriate box below. NOTE: Names are not disclosed without specific permission.

Agreement to Company Policies

• I understand that Cheerleading is a voluntary enrichment program.
• There is a $30 fee on all returned checks or echecks, as our bank charges us when a check does not clear for this reason.
• A minimum enrollment of eight children is necessary for a class to be scheduled. Classes will be cancelled with notice if the minimum is not met.
• I agree that I will not hold Stretch-n-Grow Dance responsible for accidents.


Uniform

The cost for the cheerleading uniform is $40 and includes shirt, shorts, socks, bow, pom poms, and backpack. Please indicate sizes below.
